THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Elon Health Primary Care ("we," "us," or "our") is committed to protecting the privacy of your protected health information ("PHI"). We are required by the Health Insurance Portability and Accountability Act of 1996 ("HIPAA") and applicable Florida law to maintain the privacy of your PHI, to provide you with this Notice of our legal duties and privacy practices, and to notify you following a breach of unsecured PHI. We are required to follow the terms of the Notice currently in effect.

How We May Use and Disclose Your Health Information

Treatment

We may use and disclose your PHI to provide, coordinate, or manage your health care and related services. For example, we may share information with physicians, specialists, laboratories, pharmacies, or other providers involved in your care.

Payment

We may use and disclose your PHI to obtain payment for the services we provide, such as billing your insurance company or verifying coverage and eligibility.

Health Care Operations

We may use and disclose your PHI for operational purposes such as quality assessment, staff training and evaluation, licensing, and business management activities.

Other Permitted or Required Uses

We may use or disclose your PHI without your authorization when permitted or required by law, including for: appointment reminders and treatment alternatives; public health and safety activities; reporting of abuse, neglect, or domestic violence; health oversight activities; judicial and administrative proceedings; law enforcement purposes; workers' compensation; and to avert a serious threat to health or safety. Most other uses and disclosures, including marketing, the sale of PHI, and disclosure of psychotherapy notes, require your written authorization, which you may revoke at any time.

Your Rights Regarding Your Health Information

  • Right to access: You may inspect and obtain a copy of your PHI.
  • Right to amend: You may request that we amend PHI you believe is incorrect or incomplete.
  • Right to an accounting of disclosures: You may request a list of certain disclosures we have made.
  • Right to request restrictions: You may request limits on how we use or disclose your PHI.
  • Right to confidential communications: You may request that we contact you in a specific way or at a specific location.
  • Right to a paper copy: You may obtain a paper copy of this Notice at any time.
  • Right to be notified of a breach: You will be notified if a breach of your unsecured PHI occurs.

Changes to This Notice

We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have as well as any information we receive in the future. The current Notice will be posted at our facility and on this website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
