Elon Health Primary Care is a family medicine practice on US Highway 27 in Davenport, FL, seeing patients by appointment Monday to Friday, 8am to 5pm. Whether you are establishing care for the first time, moving your records from another doctor, or you have a question about insurance, this page has every way to reach us.
The fastest route to an appointment is the phone. Call 352-508-5254 during office hours and our front desk will find you a time, usually within the same week for a new patient visit. If you would rather write, use the form on this page or email questions@elonhealthfl.com, and we will call you back to confirm.
Please do not send urgent medical questions or symptoms by email or through the form. Neither is monitored around the clock. If something cannot wait, call the office; if it is an emergency such as chest pain, trouble breathing, weakness on one side, or heavy bleeding, call 911 or go to the nearest emergency department.
We are open Monday through Friday from 8am to 5pm and closed on weekends. We are an appointment-based practice rather than a walk-in clinic, which is deliberate: it means your visit has a scheduled length, your doctor has read your chart before you arrive, and follow-up is planned rather than improvised. If you need care today for a new illness, call and ask, we hold time for established patients who get sick.
Our office sits directly on US Highway 27 in Davenport, a short drive from I-4, with free parking outside the suite and step-free access from the lot. We are convenient to Champions Gate, Haines City, Kissimmee, Four Corners, Clermont, and the rest of Polk County. If you are coming for the first time, allow a few extra minutes for new patient paperwork.
Dr. Sandeep Pandya is accepting new patients. To establish care, call or send the form and mention that you are new; we will ask a few questions about your health history and what you need so we can book the right length of visit. Bring a photo ID, your insurance card, a list of your medications, and any records from a previous doctor. Our guide to a first primary care visit covers the full list.
We are in network with Aetna, Medicare, Ambetter, Oscar, Humana Military and Tricare, and the Polk Healthcare Plan, and we are currently welcoming new Ambetter and Oscar members. If your plan is not on that list, call and we will check your coverage before you come in. Transparent self-pay pricing is available for patients without insurance; see accepted insurance and our Good Faith Estimate policy for details.
Prescription refills are handled fastest through your pharmacy, which sends the request to us electronically. Medical records requests, forms, and letters should come to the office by phone or email so we can verify who is asking. For specialist referrals, call and we will send the referral and make sure the results come back to your primary chart rather than disappearing into another practice's system.
