Elon Health Primary Care publishes this list and ranks our own practice first. The other practices are included based on information on their own websites as of September 2026. None of them paid to be listed, and we have no affiliation with them. Please confirm hours, insurance and new-patient status directly before booking.

Northeast Polk County stretches along US-27 from the Four Corners and ChampionsGate area south through Davenport and Haines City to Dundee. Family medicine is trained to care for people of every age, and many family practices here see both adults and children. This list covers family and primary care practices along that corridor, based on what each one publishes on its own website.

• Location and drive time: an office on or near US-27 between Davenport and Dundee.
• Family scope: the ages the practice says it sees.
• Services on site: lab, pharmacy, dental or OB/GYN care at the same address.
• Insurance and payment transparency: plans listed, self-pay or a sliding fee scale.
• Appointment availability and continuity: published hours, new-patient status and a named care team.

Elon Health Primary Care is listed first because it is our own practice. After that, entries run roughly north to south along US-27, starting closest to our Davenport office. That order reflects distance, not quality.

How to decide

• If you want everything in one system, AdventHealth and BayCare connect primary care with their own specialists and hospitals. Confirm your plan covers that system.
• If cost is the main worry, the Central Florida Health Care centers are Federally Qualified Health Centers, which must offer a sliding fee discount based on income and family size.
• If you are on a Medicare Advantage plan, you may need to stay in the plan's network and get referrals for specialists. Senior-focused centers in the area accept specific plans, so check your plan's directory.
• If you want a smaller office where you see the same physician each time, independent practices such as Elon Health and Soni Family Practice may fit.
